The first thing I'd try is patching in Safe Mode with Networking:

If that doesn't help, I would make a new User Account in Windows, and make it an Administrator. Then log on that User Account and try patching WoW.

You can also try turning off Vista's UAC (User Account Control):

1. Click "Start"
2. Click "Control Panel"
3. Click "User Accounts"
4. Under the User Accounts screen click on "User Accounts" (this is a second time you'll see this option)
5. Click on "Turn User Account Control on or off"
6. Uncheck "Use User Account Control (UAC) to help protect your computer.
7. Click "OK"
8. Click "Restart Now"
